How do I register with Boundaries?

You can register with us as an NHS patient if you live within our practice area which is a 5 mile radius from the surgery. You will be registered with the practice, but you have the right to express a preference to see any of the doctors. Please make this clear when you register.

Please complete a Registration Form (Adult Registration Form / Child Registration Form) and email it, along with proof of identification and address, to the surgery at NHCCG.boundariessurgery@nhs.net, alternatively come to the surgery during opening hours bringing with you some form of identification and proof of address and complete a registration form. You will need to complete a form with details about yourself and your previous GP. It is very helpful if you have your NHS card or NHS number. You can also complete the form on behalf of other members of your family.

What to do if your details change

Please let us know if your name, address or phone number changes so that we can keep your records up to date. It is also helpful to have mobile and work phone numbers where possible.

If you move to an address which is outside the Practice Area you will be asked to register with another practice whose area covers your new address. This is to ensure you can receive home visits during surgery hours and urgent care when the surgery is closed.
